GREGORY V. WARREN, M.D.
Dr Warren graduated from Mysore Medical College in 1975. He completed his Internal Medicine Residency at Christ Hospital in Oak Lawn, Illinois and completed his Nephrology Fellowship at Rush Presbyterian/St. Luke’s Medical Center, Chicago, Illinois.

In 1989, Dr. Warren established Midwest Nephrology Associates, S.C. and Midwest Dialysis Center. Since 1991 Dr. Warren has held an Assistant Clinical Professor of Medicine Appointment at the Medical College of Wisconsin. He has served on the Medical Review Committee for Renal Network 11 for four years.

Dr. Warren is Medical Director of Midwest Dialysis Center’s Ryan Road Facility. His areas of interest are chronic dialysis, kidney stones, and diabetic kidney disease.
AMIR DANIEL, M.D.
Dr. Daniel graduated from the University of Tehran Medical School in 1980. He completed his Internal Medicine Residency at Kingsbrook Jewish Medical Center in Brooklyn, New York and his Nephrology Fellowship at the University of Medicine and Dentistry of New Jersey/Robert Wood Johnson University Hospital and St. Peter’s Medical Center, New Brunswick, New Jersey.

Dr. Daniel is the Medical Director of Midwest Dialysis Center- South Facility and the Midwest Dialysis Center Peritoneal Dialysis Program. His areas of interest are acute and chronic kidney disease.

TODD MUCHE, M.D.
Dr. Muche graduated from Wayne State University School of Medicine in 1990. He completed his Internal Medicine Residency and Nephrology Fellowship at the .Medical College of Wisconsin - Milwaukee.

Since 1995, Dr. Muche has been an Assistant Clinical Professor at the University of Wisconsin - Madison Medical School and is actively involved in teaching programs for medical students and residents at the Medical College of Wisconsin and Wheaton Franciscan - St Joseph Medical Center. He has received numerous awards from both. He was also the discussant for the Nephrology section of the American Board of Internal Medicine Board Review in Chicago in 2005.

Dr. Muche joined Midwest Nephrology Associates in 1995. He is the Medical Director of Midwest Dialysis Center’s Good Hope Facility and is the Nephrology Section Chief at Wheaton Franciscan - St. Joseph Medical Center in Milwaukee.

JAMES BRANDES, M.D.
Dr. Brandes is a Native of Kenosha, Wisconsin. He graduated from the Medical College of Wisconsin in 1985. He completed his internal medicine residency and his nephrology fellowship at the Medical College of Wisconsin. Dr. Brandes served on the faculty of the Medical College of Wisconsin as an Assistant Professor of Medicine from 1990-1995 and continues to hold a clinical professor appointment at the Medical College of Wisconsin. He has received numerous awards and honors in teaching and is a past recipient of a National Institute of Health award. Dr. Brandes has spoken extensively and published topics related to dialysis and specifically to peritoneal dialysis. He currently serves as chairman of the Medical Review committee of Renal Network 11.

Dr. Brandes is Medical Director of Midwest Dialysis Center’s Capitol and Glendale facilities. He additionally serves as Medical Director and chairs Midwest Dialysis Center’s Quality Improvement Program. His areas of interest are in Peritoneal Dialysis, Hemodialysis and Hypertension.

PAUL WARREN, M.D.
Dr Warren graduated from St George's University School of Medicine in 1987. He completed his internal medicine residency and his nephrology fellowship at the Medical College of Wisconsin. Dr. Warren co-authored articles on peritoneal dialysis and is an active member in local medical societies.

Since 1996, Dr. Warren has been an Assistant Clinical Professor of Medicine at the University of Wisconsin - Madison medical school. He is Medical Director of Aurora Dialysis Center - Lake Geneva.

CLAIRE FRITSCHE, M.D.
Dr. Fritsche graduated from the Medical College of Wisconsin in 1978. She completed her internal medicine residency at the Medical College of Wisconsin and her nephrology fellowship at Boston University Affiliated Hospitals in Boston, Ma. She served as a clinical instructor at Boston University Affiliated Hospitals and was an Assistant and Associate Professor of Medicine at the Medical College of Wisconsin before joining Midwest Nephrology Associates.

Dr. Fritsche has received numerous awards, honors and research recognition and is a past recipient of the teacher of the year award presented by the Medical College of Wisconsin and the St. Luke’s Residency Program. She has published numerous papers related to Nephrology. Her areas of interest are Transplant Nephrology, Acute and Chronic Hemodialysis, and Bone Disease.

WILLIAM KRAKLOW, M.D.
Dr. Kraklow is a native of Greenfield, Wisconsin. He graduated from the Medical College of Wisconsin in 1992. He completed his Internal Medicine Residency and his Nephrology Fellowship at the Medical College of Wisconsin.

Since 1998, Dr. Kraklow has been an Assistant Clinical Professor of Medicine at the Medical College of Wisconsin - Madison Medical School. He has received numerous honors and awards in teaching. In addition to his Nephrology Renal Expertise, Dr. Kraklow is Chief of the Department of Medicine, Aurora St. Luke’s Medical Center in Milwaukee.

Dr. Kraklow is Medical Director of Midwest Kidney Care’s Kenosha and Racine Facilities. His areas of interest are Critical Care Nephrology, Acute Dialysis, and Electrolyte Disorders.

LUBNA MAJEED, M.D.
Dr. Majeed graduated from the Aga Khan University Medical College in 1990. She completed her internal medicine residency at the University of Stony Brook Health and Science Center in Stonybrook, New York. She completed fellowships in both Nephrology and Critical Care at Baylor College of Medicine in Houston, Texas. She is board certified in Internal Medicine, Nephrology and Critical Care Medicine.

Dr. Majeed joined Midwest Nephrology Associates in April of 2000. Dr. Majeed is Medical Director of Midwest Dialysis Center’s Appleton Facility. She is also a teaching attending physician at the University of Wisconsin - Madison Medical School. Her areas of interest are Dialysis, Critical Care, and Interventional Nephrology.

KATHLEEN UY, M.D.
Dr Uy graduated from the University of the East Ramon Magsaysay Memorial Medical Center, in 1993. She completed her internal medicine residency at the university of Chicago/Louis a. Weiss memorial hospital. She completed her nephrology fellowship at the University of Chicago, Chicago, Illinois.

Since 2002, Dr Uy has been an assistant clinical professor of medicine at the University of Wisconsin-Madison Medical School. Her areas of interest are acute and chronic dialysis, electrolyte disorders, and kidney stones.

VASUNDHARA GANNE, M.D.
Dr Ganne graduated from Kurnool Medical College in 1996. She completed her Internal Medicine Residency at the Chicago Medical School/Finch University of Health Sciences. She completed her Nephrology Fellowship at the Medical College of Wisconsin. She is well published with multiple abstracts in nephrology and conducting kidney stone research, while doing her fellowship. She is Board Certified in both Internal Medicine and Nephrology.

Dr. Ganne joined Midwest Nephrology Associates in October 2003. Dr. Ganne is Medical Director of Midwest Dialysis Center’s 27th Street Facility. Her areas of interest are acute and chronic dialysis, hypertension and kidney stones.

OMER AFZAL, M.D.
Dr. Afzal graduated from Allama Iqbal Medical College, Lahore and completed his residency in Internal Medicine at Southern Illinois University in Springfield, Illinois. After which he completed his Fellowship in Nephrology and Hypertension at the Medical College of Wisconsin in Milwaukee, Wisconsin. He is Board Certified in both Internal Medicine and Nephrology.

Dr. Afzal joined Midwest Nephrology Associates in 2004. He is Medical Director at Midwest Kidney Care’s West Bend and Waukesha Facilities. His areas of interest are Chronic Kidney Disease, Dialysis, Hypertension and Kidney Stones. His approach to patient care is based upon a blend of comprehensive and compassionate care for all of his patients.
DEEPAK SHARMA, M.D.
Dr. Sharma received his medical degree from the University of Rajasthan in Jodhpur, India in 1995. In 2000 he completed his residency in Internal Medicine in Jodhpur. From year 2000 to 2001 he worked as post graduate trainee primarily in Intensive Care at Escorts Heart Institute and Research Center in New Delhi. In 2002 Dr Sharma moved to the United States and joined a Masters program in Epidemiology at the Medical College of Wisconsin in Milwaukee and continued that while he completed his second Residency in Internal Medicine at the Medical College of Wisconsin from 2003 to 2006. In 2006 he received the Charles L. Junkerman Award of Merit for ‘highest standards of ethics, patient management, and respect for the dignity of an individual’ by the Medical College of Wisconsin. He went on to University of Alabama at Birmingham (UAB) in 2006 for fellowship in Nephrology. He continued at UAB, to train in Interventional Nephrology. Dr Sharma is Board Certified in both Internal Medicine and Nephrology. He is also certified in Interventional Nephrology by Divisions of Interventional Radiology and Nephrology at UAB.

Dr Sharma joined Midwest Nephrology Associates, S.C., in July 2008 and became the founding Interventional Nephrologist and Medical Director for Midwest Nephrology’s Vascular Access Center in Milwaukee.

NAIM ISSA, M.D.
Dr. Issa earned his medical degree from the Saint Joseph's University School of Medicine in Beirut, Lebanon. He then completed his internal medicine residency and his Nephrology Fellowship at the Cleveland Clinic in Cleveland, Ohio. He then completed an ASN/AST accredited Fellowship in Kidney and Pancreas Transplantation at the Mayo Clinic in Rochester, Minnesota. Dr. Issa received numerous honors and awards including recognition by the American Society of Transplantation as a Distinguished Transplant Fellow for his research in the area of kidney transplantation.

Dr. Issa presented several posters and oral communications at national and international meetings. He published several peer-reviewed articles and book chapters in the areas of Nephrology, Kidney and Pancreas Transplantation.

Dr. Issa joined Midwest Nephrology Associates in July 2008 as a Transplant Nephrologist. His areas of interest are kidney and pancreas transplantation with a particular interest in living kidney donation, renal disease in solid organ transplantation, continuous renal replacement therapies, ICU and General Nephrology. He is Board Certified in both Internal Medicine and Nephrology. He also met all criteria for the United Network for Organ Sharing (UNOS) Certification as a Kidney and Pancreas Transplant Specialist.
MARK Y. NUNAG, M.D.
Dr. Nunag was born in New York, NY. Following his graduation from the Far Eastern University in Manila, Philippines in 2001, he completed his residency in Internal Medicine at the University Hospital of New York Medical College in 2004. After accepting a position as an Assistant Professor of Medicine and Attending Physician at New York Medical College from 2005-2006, he was also appointed as the Medical Director for Westchester County's Correctional Health Services. Dr. Nunag completed his Nephrology fellowship at the University Hospital of New York Medical College in June 2008.

Dr. Nunag joined Midwest Nephrology Associates in July 2008 and is a Board Certified Nephrologist. His areas of interest are Hemodialysis, Electrolyte Disturbances, Chronic Kidney Disease and Hypertension.
THERESA BECKER, MSN APRN
Theresa Becker graduated from the University of Wisconsin - Madison in 1982 with a Bachelor of Science Degree in Nursing. She received a Master of Science Degree in Health Service Administration from Cardinal Stritch University in 1984. In 2002, she received her Master of Science in Nursing with certification as an Adult Nurse Practitioner from Marquette University. She is Board Certified as an Advanced Practice Nurse Prescriber.

Theresa's area of practice is coordinating patient care through Midwest Nephrology Associates' Chronic Kidney Disease Clinic. The purpose of this clinic is to educate patients on their renal disease. In addition, she prepares patients for kidney dialysis and transplantation. She also manages anemia (low blood count) in patients with chronic kidney disease through Midwest Nephrology’s Pre ESRD Anemia Management Clinic.
NICHOLE MELTON, MSN, APRN
Nichole Melton completed her Bachelor of Science in Nursing preparation at Marquette University in 2005. She then received her Master of Science in Nursing with Certification as an Adult Nurse Practitioner from Marquette University in 2008. She is Board Certified as an Advanced Practice Registered Nurse.

Nichole joined Midwest Nephrology Associates in 2008. Her area of practice is coordinating patient care through Midwest Nephrology Associates’ Chronic Kidney Disease Clinic. The purpose of this clinic is to educate patients on their renal disease and prepares patients for kidney dialysis and transplantation. Nichole also manages anemia (low blood count) in patients with Chronic Kidney Disease through Midwest Nephrology’s Pre ESRD, Anemia Management Clinic.
